Drug withdrawal in patients with autoimmune hepatitis in long-term histological remission: A prospective observational study
نویسندگان
چکیده
BackgroundRecommendations for drug withdrawal in patients with autoimmune hepatitis (AIH) longstanding remission are conflicting and rely on retrospective data. We prospectively investigated the predictive value of histological normalisation successful treatment AIH patients.MethodsNon-cirrhotic established complete biochemical (normalisation serum alanine aminotransferase [ALT] or aspartate [AST] immunoglobulin G [IgG]) at least 2 years were biopsied. Immunosuppressive therapy was only withdrawn (histological activity index [HAI] ≤3) a minimum follow-up 12 months.ResultsA total 17 included. Persistent inflammatory (HAI >3) precluded five patients. These had higher values ALT (25 vs. 16 U/L; p = 0.01) AST (26 22 compared remission. medication patients; eight (67%, C.I. 40-93% 0.4) remained during median 62 months (range: 13-75 months); four (33%, 7-60% required reinstitution after 1, 6, 11, 40 months, all without clinical signs disease progression hepatic decompensation. No predictors relapse identified.ConclusionTwo-thirds who prove to have achieve treatment-free Although patient numbers small results should be interpreted caution, these findings support liver biopsy prior withdrawal.

The Role of Pathological Staging in Selection of Appropriate Treatment Option for Autoimmune Hepatitis: Findings of a prospective Observational Study
Background This prospective study was designed to examine the role of fibrosis staging on selection and success of treatment options for autoimmune hepatitis (AIH). Materials and Methods The project was conducted on 110 selected AIH patients who, based on the results of liver biopsy, had been assigned into one of the three groups (mild, stages 1 and 2, moderate, stages 3 and 4, and severe, stag...
متن کاملWithdrawal of antiepileptic drugs in glioma patients after long-term seizure freedom: design of a prospective observational study
BACKGROUND Epilepsy is common in patients with a glioma. Antiepileptic drugs (AEDs) are the mainstay of epilepsy treatment, but may cause side effects and may negatively impact neurocognitive functioning and quality of life. Besides antiepileptic drugs, anti-tumour treatment, which currently consists of surgery, radiotherapy and/or chemotherapy, may contribute to seizure control as well. In gli...
متن کاملA prospective, multicenter, observational study of long-term decitabine treatment in patients with myelodysplastic syndrome.
This prospective observational study evaluated the efficacy and safety of long-term decitabine treatment in patients with myelodysplastic syndrome (MDS). Decitabine 20 mg/m(2)/day was administered intravenously for 5 consecutive days every 4 weeks to MDS patients in intermediate-1 or higher International Prognostic Scoring System (IPSS) risk categories. Active antimicrobial prophylaxis was give...
متن کاملComparison of direct and indirect blood pressure in patients under esophagectomy surgery: a prospective observational study
Background: Arterial pressure is one of the most important physiological variables and often needs to be monitored repeatedly or continuously in perioperative period. Arterial pressure monitoring is one of the standard monitoring in operating room. During general anesthesia, blood pressure can be measured by using a noninvasive arterial pressure method or continuous invasive arterial pressure b...
متن کاملOutcome of Patients with Cholinergic Insecticide Poisoning Treated with Gastric Lavage: A Prospective Observational Cohort Study
Background: Gastric lavage (GL) is one of the most commonly used decontamination method for cholinergic insecticide ingestion in developing countries despite lack of supporting evidence. This study was designed to evaluate the outcome of patients with cholinergic insecticide poisoning treated with GL in regards to timing and frequency of the procedure. Methods: In this prospective observational...
